Custom Order Forms for head pieces
Hello Pros!
I pray this finds you all well. I am in the process of creating an Order Form from which my customers can order their head pieces (hats, fascinators, hatinators, etc). Half way through, I am stuck! Do any of you have Order Forms that you will be willing to share? Any advise would be greatly appreciated.

The second link is Order Form I used for years. Others may have suggestions what else is needed.Great discussion in first link
